Of course, just after I sent this I saw a note from BartW about the 
pam_tally2 module, which does the same thing as pam_faillock.  I did a 
local test enabling pam_faillock and it worked as expected.

Seems the issue might be in pam_tally2

This may also be a duplicate of [0] which was seen before the sysadmin 
change.
